    Have you ever tried to stop drinking completely and been unable to?
    Have you ever suffered from blackouts (unable to remember; what happened whilst you were drinking) or tremors after a drinking session?
  Have you ever been involved with the police because of your drinking?
  Does your drinking cause you to have money worries
    Do you ever try to forget your troubles by drinking?
